How to Order Transcripts on Parchment

Oak Hill Alumni

To request a copy of your transcript, please fill out the Alumni Transcript Request Form. We will request the following information in the form:

• Your full name you used when you attended Oak Hill High School

• Your current legal name

• Your date of birth

• The year you graduated from Oak Hill

• Address or number you need it mailed, emailed or faxed

• Your phone number in case we need to contact you
